L
a enfermedad renal crónica en Ecua-
dor se encuentra listada como enfer-
medad catastrófica y constituye un
verdadero problema de salud pública. Su
atención genera un costo anual de cerca de
650 millones de dólares americanos, lo que
ha llevado a verdaderas dificultades econó-
micas para acceso a drogas y cobertura
1,2
.
El Instituto Nacional de Donación y Trasplan-
te de Órganos, Tejidos y Células (INDOT) es
una entidad adjunta al Ministerio de Salud
Pública, responsable de la regulación, coor-
dinación, control, promoción, sobrevida y
evaluación de la actividad trasplantológica
en el país.
La historia del trasplante de órganos y teji-
dos en el Ecuador inicia en 1964 con el Dr.
Roberto Gilbert en la ciudad de Guayaquil,
en donde hicieron el primer trasplante de
mano del mundo, siendo un procedimien-
to quirúrgico que demostró el éxito de la
técnica, aunque hubo rechazo del injer-
to, pues en esa época no se contaba con
un buen arsenal para inmunosupresión

.
El primer trasplante de riñón con donante
vivo relacionado fue realizado en 1976 en
el Hospital de las Fuerzas Armadas de Qui-
to. Desde 1976 hasta 2001, 233 trasplantes
fueron realizados tanto en hospitales públi-
cos como privados (58%), pero solo hasta
Julio de 1998 se publicó la Ley Regulatoria
y en diciembre de 1999 apareció un acuer-
do ministerial con la creación de un órgano
rector. En un estudio publicado por Huertas,
desde el año 2007 hasta el 2018 se con-
tabilizaron 211 trasplantes de hígado, 5 de
corazón, 2 páncreas-riñón, 2 páncreas, 1
pulmón, 3391 córneas, 317 de células ma-
dre hematopoyéticas. Hasta el año 2022 se
han reportado por Jiménez D y Jiménez F,
2089 trasplantes de riñón

En este ejemplar artículo titulado “Enfer-
medad renal crónica en la población ecua-
toriana y su impacto en la actividad tras-
plantológica” publicado en este número
de MetroCiencia

, los autores realizan un
estado situacional de la enfermedad renal
crónica en Ecuador y su impacto en el tras-
plante, observando que apenas el 4% de
los pacientes que se encuentran bajo tera-
pia de sustitución renal (18000 pacientes)
se encuentran en lista de espera para un
trasplante y recomiendan la necesidad de
implementar campañas de concienciación
a nivel nacional para informar a la población
sobre la importancia de la donación de ór-
ganos y alentar a más personas a conver-
tirse en donantes, entre otras recomenda-
ciones
