Refraction abnormalities in patients with stage 5 chronic kidney disease on hemodialysis

Introduction: Refractive disorders are common and correctable causes of visual impairment. It is unknown the prevalence of these disorders in patients with chronic hemodialysis and is uncertain the correction success that can be achieved.
Objectives: To know the prevalence of refractive anomalies and determine visual acuity in hemodialysis treated patients, assess the correction achieved percentage with the lenses use, compared with non-uremic patients.
Materials and methods: Case-control study of 108 patients randomized into two groups G1: uremic vs G2: nonuremic. Retinoscopy, subjective refraction and visual acuity eye using chart LogMar was performed; prevalence between groups was compared to determine the degree of damage associated with uremia.
Results: The mean age was 49 years, 54% without systemic disease. G1: 28 men (52%) and 26 women (48%), G2: 31 men (57%), 23 women (43%). Normal VA: 75% G1 vs 63% G2. Amaurosis 6% G1 vs 0% G2. The prevalence of abnormal VA in uremic patients was 25%. Astigmatism was the most frequent abnormality (G1: 51%, G2: 76%). VA recovery with correction was G1: 9% vs G2: 36%.
Conclusions: The main refractive disorders are myopia and astigmatism. Astigmatism was higher in the control group and myopia showed no significant difference between the two groups. Uremia was an independent factor in the failure of the correction of visual acuity.
