Introduction: Polycystic ovary syndrome is an endocrine-metabolic pathological condition, of multifactorial etiology, characterized by hyperandrogenism, polycystic ovaries, and/or oligo-anovulation. In addition to diverse clinical manifestations including infertility, the main objective of this research is to describe clomiphene citrate in infertile patients with polycystic ovary syndrome, through a detailed systematic review, providing a comprehensive perspective on the efficacy, safety and clinical utility of this drug. Methodology: A non-experimental retrospective observational descriptive study that collected 178 articles under inclusion and exclusion criteria in the databases PubMed, Scielo, Science-Direct and EBSCO, using a search equation formulated by keywords and Boolean operators, after which they were screened using the PRISMA methodology. Results and Discussion: Clomiphene citrate is the main therapeutic measure to treat infertility in patients with polycystic ovary syndrome, because it induces ovulation and follicular maturation, and improves pregnancy and live birth rates. However, its use for more than 6 cycles is associated with an increased risk of drug resistance, along with other risk factors such as obesity or genetic predisposition. Conclusion: Clomiphene citrate remains the first-line drug for the treatment of infertility secondary to polycystic ovary syndrome because it significantly improves ovulation, pregnancy and live birth rates.
