Objective: To compare the level of microleakage generated by restoring a MOD cavity with a fourth-generation adhesive system protocol with composite resin vs. a resin restoration with a polyethylene fiber base using a stereomicroscope and scanning electron microscope (SEM). Methodology: Class II cavities were prepared on the mesial, occlusal and distal surfaces (MOD) in 22 intact premolars extracted for orthodontic reasons. Protocol in Group A: direct resin, and group B: direct resin plus polyethylene fiber base. Samples were aged with thermocycling and stained with methylene blue. Sectioned to examine occlusal and cervical microleakage under a stereomicroscope and to measure the tooth-restoration interface in five areas using a scanning electron microscope. Results: For qualitative results, the Fisher Exact test for independent samples was used, which determined that there were no statistically significant differences between the groups. For quantitative analysis, the Shapiro-Wilk test was performed and it was determined that they have a non-normal distribution, so the statistical test for independent samples Mann-Whitney U was used through the STATA 14.0 computer program, where it was shown that there is no statistically significant difference in the measurement of the tooth-restoration interface between groups. Conclusions: Both protocols showed similar values in microleakage evaluated qualitatively and quantitatively, suggesting that either of the two protocols can be chosen in relation to microleakage
