The case of a 12-year-old female patient with a history of development of early secondary sexual characteristics is described. She came for urgent evaluation due to abdominal pain of 6 hours duration located in the left iliac fossa and with urinary symptoms. Using diagnostic aids of ultrasound and abdominal magnetic resonance, ovarian cysts measuring 5 centimeters in diameter each with a complement of tumor markers for low risk of malignancy are detected. Concomitantly, there are findings of multiple small bilateral renal cortical cysts. Due to persistence of abdominal pain despite intravenous analgesia, it was decided to resolve the condition by cystectomy and adhesiolysis by gynecology for subsequent outpatient follow-up of clinical evolution. Conclusion: Ovarian cysts are less common during adolescence and rarely occur simultaneously with renal cysts. These etiologies must be addressed when symptoms of abdominal pain or precocious puberty occur.
