Endometriosis in the previous surgery scar is a rare entity. It is defined as the presence of endometrial tissue outside the uterine cavity. Its preoperative diagnosis is a challenge due to the nonspecific symptoms that it can present with and, therefore it is underdiagnosed. Most cases occur after obstetric or gynecological surgeries. Although the definitive diagnosis is confirmed by histopathology, preoperative images such as ultrasonography, computed tomography, and magnetic resonance imaging, can be useful its assessment. We present a case report of endometriosis in a previous appendectomy scar, confirmed with a histopathological study, with a classic clinical presentation and suggestive ultrasound images.
