Introduction: The care of critically ill children in an intensive care unit allows for an improved chance of survival in life-threatening conditions. It is essential to review the actions in these units, as well as their outcomes, in search of alternatives to enhance the quality of care. Objective: To describe the clinical characteristics and outcomes of critically ill children admitted to the Pediatric ICU at Hospital Metropolitano, Quito, during the period from 2015 to 2020. Methodology: Observational study with analysis of clinical and laboratory variables collected prospectively in a database, supplemented by a retrospective review of other variables in clinical records. Results: a total of 683 children were admitted during the study period, median age of 39 months (IQR 12-89), of which 59.6% were male. The weight ranged from 3.4 to 69 kg. The length of stay ranged from 1 to 83 days, with a median of 2 days (IQR 1-5). Most frequent causes for admission were postoperative care (27.1%) and respiratory diseases (22.8%). Supportive interventions included central venous catheter placement (41.1%), arterial catheter placement (39.8%), conventional invasive mechanical ventilation (26.4%), and use of inotropes/vasoactive agents (23.3%). Ventilator-associated pneumonia occurred in 2.2% of children and catheter-related bloodstream infections in 0.9%. The overall mortality rate was 3.5%. Using PIM-3, the corrected mortality index was 0.92. Conclusion: Admission to the PICU is more frequently related to postoperative and respiratory conditions. Ventilator-associated pneumonia occurs infrequently and the overall mortality is consistent with international data.
