Liposarcoma is a rare type of mesenchymal tumor that accounts for less than 1% of all malignant neoplasms. Among these, well-differentiated liposarcoma, also known as atypical lipomatous tumor, is the most common, representing 40-45% of liposarcomas. Its risk factors are still unknown, and due to its low incidence rate, diagnostic and treatment options are limited. It is most reported in the extremities and retroperitoneum, but there are no reports of incidence in the preperitoneal fat. Therefore, there is interest in reporting a case of a 51-year-old woman with a slowly growing abdominal mass who underwent laparotomy, revealing a large preperitoneal mass, which was later histopathological confirmed as well-differentiated liposarcoma. The treatment of choice for these tumors is complete resection with negative margins and regular follow-up, as they often carry risks of recurrence. The efficacy of radiotherapy in managing these tumors is still debatable.
