Abstract

Ovarian fibromas are tumors that derive from the sexual cords, they are solid and benign in nature. Frequent complications are related to the degree of growth and its possibility of causing torsion and infarction of the ovary that can cause a lot of pain. This represents only 8% of all ovarian tumors, being even rarer that it can occur during pregnancy, having few bibliographic reports. This is a 30-year-old patient with a primigravida who comes for in vitro fertilization. However, incidentally, a mass with benign characteristics was found in the right annex, but due to the patient's desire to preserve ovarian fertility, expectant management was decided. Without a doubt, the treatment was a challenge; since she presented marked symptoms throughout the pregnancy. Treatment was limited to analgesics until full-term gestational age was reached and right oophorectomy for obvious transsurgical right ovarian torsion.
